To get started with Auscare’s CHSP services, simply follow these four steps:

1. Contact My Aged Care

Start by getting in touch with My Aged Care online or by calling 1800 200 422. Let them know about the support you need. This will trigger a Regional Assessment Service (RAS) appointment.

2. Get Assessed

A RAS assessor will visit you to understand your needs. After the assessment, you’ll receive a letter outlining the CHSP services you are eligible for.

Receive Your Referral Code

Once you are approved, My Aged Care will send you a confirmation letter with a referral code. This code gives service providers permission to begin delivering support.

Call Auscare Community Services

Contact us at 08 6364 3917 with your referral code. Our friendly team will guide you through the final steps, activate your services, and ensure you receive the right care and support tailored to your needs.
